Crockpot Nutella Hot Chocolate - only 5 minutes to prep!

6 and ½ cups skim milk (you can use milk with a higher fat content, but it will make this richer)
1 (14 oz) can sweetened condensed milk
1 tablespoon vanilla extract
1 cup Nutella
¼ cup unsweetened cocoa powder
¼ teaspoon salt
